Complete both the Mediator Certification Form (PDF Fillable Form) and Mediator Profile (Word Form), then send them by regular mail to: |

After Mr. Johns has reviewed and approved your application, he'll forward it to the court for Chief Judge Robert S. Lasnik's approval. After the court has approved your application, you are certified as a 39.1 neutral, and your Mediator Profile will be posted on the court's web site. |
